Position Summary:The Case Manager utilizes an assessment process the nursing process for age and diagnosis of specific populations to assess, plan and evaluate the care of a designated case load of patients so that clinical and financial outcomes are achieved. Displays appropriate communication skills with all guests and employees at the Hospital.Qualifications/Specifications:* Education: Graduation from a program of nursing, BSN preferred. BSW/LMSW for licensed social worker preferred.* Licensure/Certification: Current state Registered Nurse license or licensure from a state within the Nursing Compact and or State Licensed Vocational Nurse required.* Experience: 3 years clinical experience required. Two years Medical Surgical or ICU hospital experience required. One year of case management experience required.
